How can I better learn jazz piano?

I'm decent at the piano. I know my chords, scales and arpeggios. That includes blues scales and various 7th and 9th chords. However, I'm not very good at jazz piano still. I feel like there is this disconnect between knowing these things and being able to play along with an Aebersold CD or play through a fake book. What do I need to do to learn this? I just don't know what is next. Any suggestions or advice that you have would be greatly appreciated! Feel free to ask any clarifying questions, too, and I'll post further details to clarify for you.

Really there is no substitute for a real life teacher, not a lump of plastic with a screen. If you approach a local music shop they may be able to help you to find a jazz teacher, or a local jazz club. You don't need to have a lesson a week; you could have the occasional lesson on an ad hoc basis, as many teachers don't mind doing this.
I play for my own amazement...
