If you don't the title, it may be just an 'ad lib' piece of music. Jazz pianists frequently make up their own versions of a theme. You may need to contact a jazz pianist who could transcribe it for you just by hearing it - onto manuscript paper. I could do the melody, but as I am not a jazz pianist, some of the LH chords I'm unfamiliar with. There are several major 7 and major 6 chords & 'special' chords unique to jazz, and some of it is just repeated using octaves in the melody (with an acciaccatura).
